What to Bring to your First Appointment?
We want to provide all of our patients with prompt dental treatment. We request that you bring the following items to your initial visit:
- Insurance Information/Card
- Photo Identification
- Any medical records pertaining to your visit
Please arrive at our office 15 minutes before your appointment to fill out necessary paperwork. If you prefer to complete your paperwork beforehand, the appropriate forms can be found below.
At ManiDental Family Dentistry, we accept most dental insurance plans. Take some time to familiarize yourself with your insurance plan, including your benefit maximums, deductibles, and covered percentages.
We will prepare all the necessary forms for your dental benefits, but encourage our patients to remember: insurance policies are agreements between you and your insurer. Knowing the scope of your benefits can prevent surprises after your procedures and appointments, from emergency appointments to general dentistry services.
Please know that you are responsible for all dental fees, should your insurance benefits result in less coverage than expected. We do not provide our services with the understanding that insurance companies will pay all of our fees. We base the delivery of our services on which treatment options are best for you. Ultimately, your dental benefits are contracted between you or your employer and your insurance company.
You’ll find our new patient forms below. Feel free to print and complete the forms before your first visit with us. We do not require you to complete paperwork prior to visiting our practice, but doing so will save time for you and our staff.
You’ll need Adobe Acrobat Reader to review the forms. You can download a copy of Adobe Acrobat Reader here.
Payment is expected at the time services are rendered, unless other arrangements have been made with our office. This includes copayments or deductibles (the amount not covered by your insurance).
For your convenience, our office currently accepts cash, personal checks, and credit cards.
