Your first visit to ManiDental establishes a vital foundation for our relationship with you. 

During your first visit, we will secure vital background information, including your medical history. We will also give you plenty of time to get to know your doctor and ask questions.

Making Appointments

Our office is currently accepting new patients. Please call our office at 336-835-3337 to schedule an appointment and reserve your spot. If, for some reason, you are unable to attend your appointment, please call us within 24 hours to reschedule.

What To Bring To Your First Appointment

We want to provide all of our patients with prompt dental treatment. We request that you bring the following items to your initial visit:

  • Insurance Information/Card
  • Photo Identification
  • Any medical records pertaining to your visit

Please arrive at our office 15 minutes before your appointment to fill out necessary paperwork. If you prefer to complete your paperwork beforehand, the appropriate forms can be found below.

Insurance

At ManiDental Family Practice, we will gladly help you by filing your dental insurance claims. Insurance is ultimately a contract between you and the insurance company. Take some time to familiarize yourself with your insurance plan, including your benefit maximums, deductibles, and covered percentages. Please note that we no longer accept Medicaid insurance.

You are responsible for providing us with the correct information to file your claim. Some companies change benefits at the beginning of the calendar year. You may be notified of these changes by mail or by your employer. Please show us any new cards or information you receive so we may help you get all the benefits that you are entitled to.

If your insurance does not pay 100% of the amount due, you will be responsible for any balance remaining. We do not provide our services with the understanding that insurance companies will pay all of our fees. We base the delivery of our services on which treatment options are best for you. Ultimately, your dental benefits are contracted between you or your employer and your insurance company.

Payment

Payment is expected at the time services are rendered, unless other arrangements have been made with our office. This includes copayments or deductibles (the amount not covered by your insurance).

For your convenience, our office currently accepts cash, personal checks, and credit cards. We also now accept Sunbit, which allows you to pay for your dental services over time through affordable, low payments.

You’ll find our new patient forms below. Feel free to print and complete the forms before your first visit with us. We do not require you to complete paperwork prior to visiting our practice, but doing so will save time for you and our staff.
